Q: What would fail a pre-employment physical?

There's no universal pass or fail. Your employer sets the standard for the job, and the exam confirms you can do that specific job safely. The most common reason a form can't be completed isn't the exam — it's a test or document the employer required that wasn't done.

Q: Will my form be completed the same day?

Yes. If your employer requires lab tests, we sign it with those results marked pending, so you can start work.

Q: How long is an employment physical good for?

Your employer decides. Most accept an exam done within the last 30 to 90 days. Check your form or ask your employer.

Q: Should I fast before the exam?

No. Eat normally, and bring your employer's form.

Q: Can I refuse a pre-employment physical?

That's between you and your employer. Under federal law, an employer can require a medical exam only after making a job offer.

What's included in the $150 exam

Employers require a physical for a job to confirm you can do that job safely — and, in fields like healthcare, construction, and childcare, to meet industry rules. This is not an annual physical. It's a focused, job-related exam.

We don't accept health insurance for anything done in our office. The lab can bill your insurance for blood and urine tests; if you have none, it bills you directly at the prices above.

We take cash, debit, and credit cards. A 3.5% processing fee applies to credit card payments; there's no fee for cash or debit.

If you later find out your form needs a test you didn't get, come back. You pay only for that test. There's never a second office fee.

We examine you, place the PPD, give any vaccines, draw blood, and sign your form. If your employer requires lab tests, we sign the form with those results marked pending, so you can start work while Northwell processes them. The exam itself takes about 10 minutes; plan on 30 minutes for the whole visit.

Vaccines and immunity titers

Healthcare jobs and many other employers require proof you're immune to measles, mumps, rubella, and chickenpox (varicella), a current Tdap, and often hepatitis B.

There are two ways to prove immunity: a blood test called a titer, which shows you're already immune, or a record of the vaccine itself.

We give them at the visit.

Drug screening

We collect the sample here for a 10-panel urine drug screen — it checks for ten types of drugs — and Northwell bills it. We don't offer alcohol testing.
